Welcome to the Family Care Center! It is our goal in this office to serve you in the best possible way we can. To do this we try to keep our cost at a minimum and your visits here as stress free as possible. It is our office policy that you come prepared to pay your bill each time you see your therapist. We accept cash, checks, MasterCard, and Visa. Thank you for helping us reach our goal to serve you better. Release of Information

If you are in need of copies of your records, please fill out the forms below and mail for fax them to us. Legislation has been passed regarding the fees for mailing records and that the first ten (10) pages will be $15, plus $0.25 for every additional page. FCC will not charge for the postage related to mailing the material. If you have any questions, please contact us at (260) 493-0055.
